* Our membership fee covers the costs of all uninsured services included in our year-round care membership (a list of which can be found here). We do not charge for insured services, which are services covered by OHIP. OHIP covered services are available to all OHIP card holders regardless of whether a membership package is purchased

* Eligibility for reimbursement under a Health Spending Account (HSA) is subject to your specific plan provisions, coverage, available balance, and eligibility conditions. Check with your insurance provider to confirm eligibility and reimbursement criteria. Platinum Medicine will provide you with a receipt for services rendered which will include a description of the service, the provider’s name and credentials, and the date and cost of service. Reimbursement is at the sole discretion of the insurance company, and outcomes can vary depending on the insurance provider
